New Student Health


Medical History Form

All new and transfer students must complete a Medical History Form prior to moving into the residence halls and starting classes.

Here's how to access the form on our Student Wellness Portal:

  1. Log into the GMercyU Portal
  2. Click the Student Wellness Portal icon to launch our Student Wellness Portal.
  3. First-time users will need to click "Register" and enter your information. For username, enter your GMercyU Portal username (usually lastname.firstinitial). For email, enter your GMercyU email address. 
  4. Once logged into the Student Wellness Portal, click "My Forms".
  5. Click "Gwynedd University Medical History Form".

If you have trouble logging into the Student Wellness Portal, please email our IT department at IT@soongshinkid.com Monday-Friday, 8:00 a.m. to 4:00 p.m.


Athletic Student Health Form

If you need to fill out the Athletic Student Health Form, please contact the Athletic Department at 215-646-7300, ext. 21251.


Nursing Student Health Form

If you are enrolled in the four-year BSN program, but not enrolled in any nursing courses for the fall semester, complete the regular University health form. More documentation will be provided to you in the beginning of your sophomore year by the Frances M. Maguire School of Nursing and Health Professions.

Required Immunizations

The following immunizations are required for all students. Non-compliance will result in a medical hold for admission to the residence halls and may interrupt your class schedule.

  • DPT: 3 doses plus a booster within the past 10 years
  • Polio: 3 doses of Polio vaccine plus a booster
  • MMR: 2 doses
  • Hepatitis B: 3 doses
  • Meningococcal Meningitis Vaccine: If you plan to live in the residence halls, this vaccine is required.
  • Meningococcal Meningitis Vaccine Booster: A booster dose is now recommended at 16 years old for children who received the initial dose between 11 and 12 years old. If the initial dose was given between 13 and 15 years old, the booster dose should be given between 16 and 18 years old. If the initial dose was given at 16 years or older, no booster is needed, except where there is continuing risk.
  • Varicella: either a history of chickenpox or the varicella vaccine
  • TB test: A PPD within one year for all nursing, Health Professions, International, and Resident Students. Nursing Students are to be repeated yearly. Health Professions students will follow recommendations from their respective departments.

Insurance Coverage

February 2024 update

All full-time campus undergraduate students, ABSN, DEMSN, MSOT, and MS SLP students are required to have medical health insurance. Resident students are required to provide proof of health insurance coverage on their health forms in order to live on campus.

If you do not have your own insurance, here are some outside resources that you can access to find a plan that meets your needs:

GMercyU's Student Financial Aid Office will work with students who need financial assistance for outside health insurance coverage. Please contact Financial Aid at financialaid@soongshinkid.com or 215-646-7300 ext. 21216, if needed.

Meningitis Awareness Disclosure

On June 28, 2002, the Governor of Pennsylvania, Mark Schweiker, signed into law a requirement that students enrolled in Pennsylvania institutions of higher education residing in residence halls or other university-owned housing be vaccinated against meningococcal disease. Monkey Pox

The Health and Wellness Center is closely monitoring developments around monkey pox and will respond to our community needs accordingly. For the most up-to-date information, please review resources at U.S. Centers for Disease Control (CDC) and www.montcopa.org.

If you are experiencing symptoms that might be monkey pox, including fever, headache, muscle aches and backache, swollen lymph nodes, chills, exhaustion, and a rash that can look like pimples or blisters, it is essential that you:

  • Immediately isolate yourself from others; do not report to work or school until evaluated and cleared by a healthcare professional.

  • All students should immediately contact the Health and Wellness Center at healthandwellnessctr@soongshinkid.com.

  • Resident students should stay in their room and await further instructions from the Health and Wellness Center.

  • Commuter students should not proceed to campus until cleared by their primary care physician.

  • All faculty and staff should email the Health and Wellness Center at healthandwellnessctr@soongshinkid.com so they are aware of possible/actual cases.

Anyone who is made aware of a positive case, suspected case, or is in close contact with monkey pox should immediately notify the Health and Wellness Center at healthandwellnessctr@soongshinkid.com.
